Medication wallet card maker

In an emergency, a card in your wallet tells paramedics and doctors what you take. Fill it in, print it, fold it into your wallet.

Medications

Runs in your browser. Nothing you type leaves this page and nothing is saved.

Close the page and it is gone, so print it before you leave.

Keep it up to date: print a new card whenever a medication or dose changes.

What to put on it

The front holds who you are, your allergies, any conditions that matter in an emergency, someone to call and your doctor. The back lists each medication with its strength and how you take it, as the label says. Include vitamins, supplements and anything you take only when needed, since they can matter to a doctor too.

The printout puts both sides at standard card size, 85.6 by 54 mm, with a full-page list underneath for a GP visit or a hospital bag. Cut out the two sides and glue them back to back, or fold along the edge. A clear card sleeve keeps it readable.
