Pick one time you can keep every day, then build the routine around it. For levothyroxine, the NHS says to take it on an empty stomach 30 to 60 minutes before your first meal of the day, any drink with caffeine or any other medicine, and to try to take it at the same time each day.
This guide is about the routine and not about your dose. Always follow your own label and your doctor's directions, because yours may differ.
Why the routine matters so much
Levothyroxine controls an underactive thyroid but does not cure it, so MedlinePlus says to keep taking it even if you feel well, and not to stop without talking to your doctor. It may also take several weeks before you notice a change, so how you feel is not a reliable reminder.
The usual morning problem
The trouble is that the wait falls right when you want breakfast and coffee. Some ways around it:
- Put it by the bed with a glass of water, if your morning allows, and take it before you get up. Keep it safely out of reach of children.
- Set the coffee aside. Put the tablet box beside the kettle so you meet it first.
- Start a timer for the wait when you take it. Then you are not watching the clock or guessing.
If mornings really do not work, ask your doctor or pharmacist about alternatives rather than changing the time by yourself.
Plan the gaps from other things
Some medicines and supplements can get in the way. The NHS lists antacids and other indigestion treatments, calcium and iron supplements, and biotin (which can affect thyroid tests) among those that may not mix well. MedlinePlus says to take calcium carbonate or ferrous sulfate at least 4 hours before or after levothyroxine.
These are examples, not a full list. Show your pharmacist everything you take and settle the times once, so you do not have to work them out each day.
If you forget
Check your leaflet. The NHS says to take it as soon as you remember unless it is nearly time for your next dose, in which case skip the missed one. It says not to take 2 doses at the same time. Ask your pharmacist if you are unsure. See what to do if you missed a dose.
Stop wondering whether you took it
Because it is the same small thing every morning, it is easy to forget whether you did it today. A log settles that. See did I take my medication?
A weekly pill box or a reminder that follows up if you don't respond can help. Pick whichever you will really use: here is how they compare.
