Where Cadence excels
No subscription
Cadence is free with an optional one-time Pro upgrade for $9.99, with a 7-day free trial. Medisafe charges $40 per year. Over five years, that is $200 versus $9.99. Over ten years, $400 versus $9.99. For a tool you may need for decades, the pricing model matters.
Privacy-first design
Your medication data stays on your device and syncs securely through your own iCloud account. No account creation required, no data sent to third-party servers. Medisafe requires an account and stores your data on their cloud infrastructure.
Prescription label scanning
Point your camera at a prescription bottle and Cadence reads the label, matches it to a database of 2,300+ medications and fills in the name and dosage automatically. This feature is not available in Medisafe.
Apple Health correlations
Cadence Pro connects to Apple Health and pulls in 10 health metrics -- heart rate, resting heart rate, blood pressure, respiratory rate, blood oxygen, steps, weight, blood glucose, sleep and HRV. It displays these alongside your adherence data so you can spot patterns and correlations over time. Medisafe does not offer health metric correlations with adherence data.
Apple Watch app
Cadence includes a dedicated Apple Watch app for logging doses, viewing your schedule and receiving reminders on your wrist. Medisafe does not have an Apple Watch app.
Journey visualisation
The Journey feature turns your medication routine into a visual mountain trail. Each day you stay consistent, the landscape shifts with new seasons, milestones and heights over a full year. It is a motivational layer that rewards consistency without being pushy.
Where Medisafe excels
Caregiver sharing
Both apps offer a way to keep family or caregivers in the loop. Cadence ships a caregiver report that lists each medication with on-time, late, missed and upcoming statuses, ready to share. Medisafe's MedFriend feature takes a different approach by automatically push-notifying a chosen contact the moment a dose is missed. If real-time push to a caregiver is essential for your situation, Medisafe still has the edge there.
Drug interaction checking
Both apps check for known interactions between your medications. Cadence ships a bundled offline interaction database that runs entirely on your device, so your medication list is never sent to a server. Medisafe runs its checks against a cloud-based service. Both flag potential conflicts. Cadence's approach keeps your list private by default.
Android support
Medisafe is available on both iPhone and Android, making it the better choice for Android users or households with a mix of devices. Cadence is available on iPhone and Apple Watch only.
Track record
Medisafe has been on the market since 2012 and has a large, established user base. It has gone through many iterations and is a proven product. Cadence is newer and still expanding its feature set.
Pricing over time
| Timeframe | Cadence Pro | Medisafe |
|---|---|---|
| Year 1 | $9.99 | $40 |
| Year 3 | $9.99 | $120 |
| Year 5 | $9.99 | $200 |
| Year 10 | $9.99 | $400 |
If you are managing a chronic condition, you are likely taking medication for years or decades. The pricing model you choose today is the one you will be paying for the foreseeable future.
Which app should you choose?
Choose Cadence if you are an iPhone user who values privacy, wants to see how your medication habits correlate with your health data and prefers paying once rather than subscribing. The Apple Watch app, label scanning and Journey feature are additional advantages if you are in the Apple ecosystem.
Choose Medisafe if you need real-time caregiver push notifications the moment a dose is missed, or you need an app that works on Android. The subscription cost is worth it if those caregiver push features are essential for your situation.
Both are solid medication reminder apps. The right choice depends on what matters most to you -- and neither is wrong.
